Before Your Visit
Being well prepared for the first and subsequent visits is very important. The following list serves as a guide for what to bring for a complete evaluation of the child:
- Birth history
- Developmental history (i.e., age started to seat, walk, talk, etc.)
- Past surgical history
- Any medical reports from previous evaluations
- List of current medications
Also, if your child has any braces or adaptive equipment, please bring them to the evaluation.
